Who is Eligible?
To enroll in Ascension Living St. Vincent PACE, individuals must be:
- Age 55 or older
- Living within our St. Vincent PACE service area
- Certified as meeting Indiana criteria for nursing home level of care
- Able to live safely in community with the support of St. Vincent PACE
How do I join?
Call us at 463-271-3700 to speak with our intake staff, who will arrange a personal visit with you in your home or in our Adult Day center, explaining the program in more detail. Once you have decided to join the program, we will arrange a visit to the St. Vincent PACE center, where you will meet our dedicated staff of professional caregivers who can assist you in outline your personal plan of care. Ascension Living St. Vincent PACE is a voluntary program, and once enrolled, you may choose to disenroll at any time by contacting your St. Vincent PACE social worker. How to refer someone
Anyone can make a referral or reach out to St. Vincent PACE to get started. Physicians, social workers or other professionals can refer someone to PACE, using one of the options below. When PACE receives a referral, we reach out to the caregiver or potential participant to provide information and answer questions.

Where is St. Vincent PACE located?
The St. Vincent PACE center is located at 5435 W. Pike Plaza Road, near W. 38th Street in Indianapolis, IN.
How would I get to Ascension Living St. Vincent PACE and what would I do there?Transportation is available by Ascension Living St. Vincent PACE. One of our wheelchair accessible vehicles can pick you up at home and bring you into the adult day center for the day. Depending on your personal needs, you may be seen by clinic nurses, physicians and therapists. You will receive a hot noon meal, plus social and recreational activities with other participants.
What happens to my Medicare/Medicaid coverage when I join?
Participants with Medicare and/or Medicaid do not lose their coverage, but authorize Ascension Living St. Vincent PACE benefit administration. This means you maintain Medicare and/or Medicaid benefits, gaining additional Ascension Living St. Vincent PACE benefits. All services are provided under one umbrella so you do not have to worry about "falling through the cracks" with hidden costs or deductions.
If eligible, participants agree to receive all needed health care services from Ascension Living St. Vincent PACE or our contracted provider. Cost for unauthorized or out-of-network services may be the responsibility of the participant. Other than emergency services, all services must be approved by the Ascension Living St. Vincent PACE Team.
What is the cost?
What you pay for St. Vincent PACE depends on your eligibility for Medicare and/or Medicaid. Participants enrolled in both Medicare and Medicaid, or just Medicaid alone, have little or no cost for any care or services within their care plan. Participants enrolled in both Medicare and Medicaid, or just Medicaid alone, have no copayments or deductibles for anything within their care plan. We can assist with new Medicaid applications. With Medicare alone, there is a cost-sharing option.
General Policies and Information
PACE participants must meet the level of care requirements according to Indiana guidelines. Ascension Living St. Vincent PACE is funded by Medicare, Medicaid and private pay. As a Medicaid recipient, you may have little or no financial obligation.
All PACE participants agree to receive all services from St. Vincent PACE staff and its network of providers and may be fully and personally liable for the costs of unauthorized or out-of-network services. Ascension Living St. Vincent PACE specializes in comprehensive healthcare for our participants and, as part of our program, is pleased to provide the following resources for our participants.
Ascension Living St. Vincent PACE is a voluntary program, and once enrolled, you may choose to disenroll at any time by notifying your St. Vincent PACE social worker.
