Cybersecurity Event Update

Ascension previously reported that it experienced a ransomware attack that has caused disruptions to patient care in its network. Our priority remains on providing safe patient care. Ascension, with the support of leading cybersecurity experts, worked around the clock over the weekend to respond to the ransomware incident affecting our systems. We are focused on restoring systems safely. We are making progress, however, it will take time to return to normal operations. As systems and services come back online, we will share those updates so that our patients and communities can plan accordingly. We have established a dedicated website to share any updates we have across our system.

How to Enroll

Do You Have to Pay for PACE?

There is no fee if you are financially qualified for TennCare long term services and supports.If you are eligible for Medicare only, there will be a fee involved. Those without Medicare or TennCare may pay privately. The PACE enrollment coordinator will help you apply for TennCare.

All services must be approved by the PACE team. Cost for unauthorized or out of network services may be the responsibility of the participant.

Who is Eligible?

INDIVIDUALS WHO ARE:

Age 55 or older
Living in Hamilton County, Tennessee
Assessed by the PACE interdisciplinary team as being appropriate for PACE services
Certified by TennCare as eligible for nursing home level of care

How to Refer Someone

Physicians, social workers, professionals and anyone else can refer someone to PACE, as long as they ask the potential participant or caregiver for permission to do so. When PACE receives a referral, we reach out to the caregiver or potential participant to provide information and answer questions.
