PACE participants must meet the level of care requirements according to Kansas guidelines. Ascension Living HOPE is funded by Medicare, Medicaid and private pay. As a Medicaid recipient, you may have little or no financial obligation.

All PACE participants agree to receive all services from HOPE staff and its network of providers and may be fully and personally liable for the costs of unauthorized or out-of-network services. Ascension Living HOPE specializes in comprehensive healthcare for our participants and, as part of our program, is pleased to provide the following resources for our participants.

Ascension Living HOPE is a local, comprehensive program of healthcare and support services that has been coordinating and providing medical care for Sedgwick County seniors since 2002. Ascension Living HOPE is based on the growing, national model of care called Program of All-Inclusive Care for the Elderly (PACE). 

The goal of PACE is for you to enjoy the quality of life you desire while supporting your needs and safety in the place you call home. PACE focuses on helping you remain healthy and safely in the community as long as possible. When you receive services from Ascension Living HOPE, you will work with a local team of healthcare providers that strives to know you on a personal level, working with you to create an individualized care plan based on your lifestyle and goals. Their team includes medical, nursing, social work, therapy and other professionals who work together at their center to promote quality services, effective care and provide ongoing support for you.

The Ascension Living HOPE Adult Day Health Center is the hub for most services. Some services — such as primary medical care, lab tests, therapy, social activities and more — will take place on-site. Others — such as transportation, in-home services, specialty care and more — may be provided outside of the center, but are still coordinated by your care team through the day center.

As you consider which plan may be right for you, we want to provide you with information on all of your options, allowing you to make an informed decision. Ascension Living HOPE is a voluntary program, and once enrolled, you may choose to disenroll at any time. Q. Who is eligible to enroll?

You must be:

  • 55 years of age or older
  • A resident of Sedgwick County
  • Eligible for nursing home placement according to Kansas Department on Aging guidelines
  • Able to live safety in the community without jeopardizing your health and safety

If eligible, you must agree to receive all medical and community services from Ascension Living HOPE's staff and network of providers


Q. What types of payment does HOPE accept?

Payment is received through Medicare, Medicaid and private pay. Most people will have only a minimal monthly obligation


Q. Where is Ascension Living HOPE?

The HOPE Center is located in West River Plaza, the former site of Riverside Hospital, at Central and McLean. Staff is on call 24 hours a day, seven days a week, including holidays.


Q. What happens to my Medicare/Medicaid coverage when I join?

Participants with Medicare and/or Medicaid do not lose their coverage, but authorize Ascension Living HOPE benefit administration. This means you maintain Medicare and/or Medicaid benefits, gaining additional Ascension Living HOPE benefits. All services are provided under one umbrella so you do not have to worry about "falling through the cracks" with hidden costs or deductions.  


Q. How would I get to Ascension Living Hope and what would I do there?

Transportation is provided by Ascension Living HOPE. One of our wheelchair accessible vehicles will pick you up at home and bring you into the adult day center for the day. Depending on your personal needs, you may be seen by clinic nurses, physicians and therapists. You will receive a hot noon meal, plus social and recreational activities with other participants.


Q. How do I join?

Call us at 316-844-1350 and speak to our intake staff, who will arrange a personal visit with you in your home or in the Adult Day Center, explaining the program in more detail. Once you have decided to join the program, we will arrange a visit to the HOPE Center, where you will meet our dedicated staff of professional caregivers who can assist you in outline your personal plan of care.
