Live at Home is a membership program that gathers the right resources to help you live at home through life’s later phases, with options to help manage every aspect of daily life – from medical to home care to transportation and more. Your home is your place of comfort, and a change in your health or mobility shouldn’t take that away – whether it’s a short or long term change. As a Live at Home member, you can feel confident and secure knowing you have the services and access you want to enjoy life – at home.
Live at Home Membership starts with a LiveWell Plan:
We tailor our membership packages to your unique needs - so every membership subscription starts with a LiveWell Plan. We spend quality time with you and your loved ones to truly understand your home and your life. With an in-depth consultation by our Wellness Coordinators, we can create an easy-to-follow plan that keeps you in control, and is updated along the way so it’s always right for you. Each of our services is a la carte, so you know you’re getting exactly what you need and nothing you don’t.
A Live at Home Membership includes 24/7 concierge services for:

What does it cost to be a member of the program?
Membership costs vary based on what level of service you prefer, and if you prefer to pay monthly or annually. Like other membership programs, paying annually can save you money, but we offer both options to give you flexibility. Contact us to learn more about plan pricing.
When is the concierge line available?
The concierge line is open 24 hours a day, 7 days a week. We are available when you need us.
I know what I need – do I still have to start with the consultation?
Yes. Even though you have a handle on your current needs, our in-home consultation and Live Well Plan goes over a comprehensive list of topics and ensures everyone is on the same page. You can include loved ones in the consultation – we want to create open communication so you have what you need.
Short or long-term, the LiveWell Plan gives everyone clarity and peace of mind on the path forward.
What is the process of signing up?
Fill out the form online, or give us a call. A Wellness Coordinator will schedule a time to meet you in person, at your home, for your LiveWell consultation. Once complete, we will follow up with you to deliver and go over your LiveWell Plan. Your plan will include membership options for you – you decide what you prefer and we will opt you in to that membership level.
What if I only need this support for a short time?
No problem. We offer three packages, so you can choose the time frame that fits your needs: one, six or 12 months. Oftentimes, when someone is recovering from a procedure or undergoing physical therapy, an extra boost of support for daily tasks can be a big help in recovery.
Can your transportation service accommodate a wheelchair?
Yes. Our transportation service is equipped to assist you and any mobility device you use. We understand the needs of those with mobility devices and are ready to support you, so you can stay mobile and connected.
