Meet Evelyn

Evelyn enrolled with Ascension Living HOPE about five years ago. She knew a couple of HOPE participants and they recommended that she look into HOPE, so Evelyn decided to check it out. Evelyn’s medical conditions made it difficult for her to complete household tasks, so she needed help around the home and she needed transportation services to get to her appointments. After enrolling with HOPE, Evelyn started coming into the day center more frequently and she would participate in many different activities and socialization events at HOPE. Evelyn is still living in the community independently, with support from HOPE.

Meet Laura 

About 4 years ago, Laura’s husband called Ascension Living HOPE looking for information. Alice was starting to need more care and supervision during the day but both Laura and her husband worked during the day full-time, so they were limited in the support they could provide. They found HOPE by searching the internet for day programs and reached out to us. Alice enrolled at the start of the following month and started coming into the HOPE day center and receiving HOPE services in her home. Fast forward to today and Alice is still living in the community with her daughter and son-in-law, with a little support from HOPE.

Meet Perry

After a major stroke, Perry’s brother Wendell didn’t know where to turn. He and his family tried to help, but it was overwhelming. They were relieved when Perry was eligible for PACE Michigan’s one-stop, all-inclusive care. Sports were always a big part of Perry’s life before his stroke. Within a short time with PACE, Perry started physical therapy and regained some of the strength he had lost with his stroke.  

After living 5 years in his apartment, PACE knew Perry needed a higher level of help.  PACE worked with Perry and his family to move him into assisted living.  Wendell shared “the transition was fine and went well. Perry is doing a lot better since the move.  He's not sleeping as much, he's more active and benefiting from being around others.”  Perry continues his journey in his new home with his family and PACE oversight.

Meet JoAnne

JoAnne moved in with her daughter after her husband died. She was quiet and showed little emotion during her first visit to PACE Michigan. Now, she is excited and feels more independent stepping onto the PACE bus. She looks forward to time at PACE with friends and keeping busy. There is peace of mind for her daughter when she’s at work. Mom’s care is overseen by caring staff and she’s safe. Plus, JoAnne’s energy and interests have bloomed again.

Meet Phyllis

“During 2020 I didn't get to go to the day center because of COVID precautions and I missed my buddies, but my home care that Ascension Living PACE provides is fantastic. I had CNAs three times a day to keep me clean and make me laugh. They kept my spirits up. The CNAs were in their mask and gloves as soon as I answered the door. I’m getting physical therapy at home and they fixed my wheelchair and bed.

Whatever I need, Ascension Living PACE provides.  My medication is delivered to me by Ascension Living PACE. The activities director and social workers call me regularly to see that I’m alright. I trust Ascension Living PACE staff will keep me safe -- you gotta have trust.

My favorite thing about Ascension Living PACE is the way you all treat me. You give me respect, care, and friendship. You tell me things straight. I wouldn’t be able to live in my own home if it wasn’t for Ascension Living PACE.

Meet Cassandra

“I feel confident the staff at Ascension Living PACE is keeping us safe. I first and foremost appreciate that Ascension Living PACE closed the day centers down  to keep the elderly safe when COVID hit.  Ascension Living PACE didn’t miss a beat providing services in my home.

When the CNA’s come to my home to provide care to my mom they wear masks and gloves the whole time they’re here. I have not missed a medicine refill or incontinence products and Ensure delivered by Ascension Living PACE transportation. We get calls once a week from the activities director and the bus driver to check in and see how mom is doing. To me, it’s heartfelt and we feel completely blessed.

I want to keep my mom home with me; I don’t want her to go to a nursing home. With the support of Ascension Living PACE I can keep her home.  PACE is a Godsend.”
