Lisa Musgrave
Senior Vice President, Post-Acute and At-Home Services

As Ascension’s Senior Vice President of Post-Acute and At-Home Services, Lisa Musgrave, MHA, BSN, RN,  provides senior management and leadership to Ascension Living, one of one of the nation’s leading nonprofit and Catholic senior living organizations; Ascension at Home, a home health division managed in partnership with Compassus Hospice and Home Health; Program of All-Inclusive Care for the Elderly (PACE) services; and inpatient/outpatient rehabilitation. 

Prior to joining Ascension, Lisa served as Vice President, Post-Acute Services & Care at Home, with AdventHealth, with responsibility for developing and administering strategic, operational and financial objectives for post-acute and home-based services, post-acute care collaborative networks, and clinical aspects of the system’s home care and hospice central billing office. In this role, she led the development of new high-acuity home services such as advanced, acute and extended care at home.

Prior to serving in AdventHealth, Lisa served as Senior Vice President of Care Solutions for Navvis Healthcare based in St. Louis, with responsibility for developing population health/care management platforms and building community networks to bridge the space between payers, providers and patients. She also served as Vice President, Home Based Care, for Memorial Hermann Health System in Houston and as Chief Nursing Officer/Operations Officer for Home Health and Hospice at Intermountain Homecare, a division of Intermountain Healthcare, Salt Lake City. Earlier she held healthcare leadership positions in Tulsa, Oklahoma; Kansas City, Missouri; and Columbia, Missouri.

Lisa is a Registered Nurse who earned a Bachelor of Science in Nursing from William Jewell College, Liberty, Missouri, and a Master of Health Administration from Ohio University, Athens, Ohio. She has served on the Missouri State Home Care Board and was a volunteer for the Meals on Wheels Board. She is an active member of her church and has served in youth and children's ministry for more than 30 years. She also has participated in medical and humanitarian missions in Mexico, Central and South America.

Rachel Raymond
Chief Strategy Officer, Horizontals
Rachel oversees strategy for all Ascension horizontals including Ambulatory Surgery Centers, Imaging, Ascension Rx, Behavioral Health, Urgent Care, and Post-Acute and At-Home Services, encompassing Rehabilitation and Ascension Living.  

As the Chief Strategy Officer, Raymond develops and implements strategies designed to advance and further integrate post-acute services into Ascension’s healthcare continuum. This includes services internal to Ascension and those provided by outside partners. She is positioning Ascension to deliver new models of care in the home through high-quality preferred provider networks in conjunction with exceptional at-home partners. Her work drives improved quality, reduces total cost of care, facilitates intense provider collaboration and enables seamless transitions for consumers. 

Since joining Ascension in 2014 she also has served as Associate Vice President of Strategy and Associate Vice President for Strategic Solutions within Ascension Care Management. Before launching her healthcare strategy career in Nashville, Tennessee, Rachel spent seven years in policy, strategy and program management roles in Washington, D.C., designing and implementing national programs, including state health insurance exchanges across the country. 

She earned a master’s degree from Vanderbilt University in Nashville and a bachelor’s degree from the University of Tennessee. Rachel is a member of the Ascension Leadership Academy and was named a”40 under 40 leader” by Nashville Business Journal.

Erin Shadbolt
Chief Operating Officer, Ascension Living Chief Nursing Officer, Post Acute and At Home Ascension

Erin is an experienced healthcare leader skilled in post-acute care operations, finance, growth, regulatory compliance, quality improvement and strategic planning. She has a proven track record of implementing new programs, improving financial outcomes, increasing census, developing critical relationships, successfully completing regulatory surveys, improving quality metrics, and improving staff and patient engagement.

Erin came to Ascension from Mayo Clinic Health System, where she served as Nurse Administrator. In that role, she was responsible for implementation of Advance Care at Home (hospital at home) and had oversight over home health, hospice, senior services in skilled nursing facilities, and palliative care in northwest Wisconsin. 

Erin received her Associate of Science degree in Nursing, Bachelor of Science degree in Nursing and Master of Science degree in Nursing from Weber State University. She also is a Nursing Fellow of the Golisano Institute for Intellectual and Developmental Disability Nursing at St. John Fisher College, New York, New York and is currently completing a certificate through Indiana University in Healthcare Innovation and Implementation.

Bob Smoot
Chief Mission Integration Officer

Bob Smoot began his service as Chief Mission Integration Officer for Ascension Living in December 2018, having previously served as the Senior Vice President and Chief Mission Officer for AMITA Health, the Chicago-based joint venture of Ascension’s Alexian Brothers Health System and Presence Health with Adventist Midwest Health.

In this pivotal role, Bob is responsible for establishing a collaborative framework for the growth of a common culture across Ascension Living, building upon the richness of the healing traditions of our legacy organizations and advancing our ministry integration grounded in our Mission and Values. His team works to deepen the awareness of and fully realize our identity as a healing ministry of the Catholic Church in all aspects of strategy, organizational culture, service and community collaboration and promote Ascension’s broad strategic and organizational commitment to our Mission. Bob assures the evolving initiatives, programs and resources that support the One Ascension journey, engaging the gifts of all for the sake of our Mission.

He brings a wealth of knowledge as a thought leader in the nuanced application of ethics, spirituality and community transformation with special attention to those who are poor and marginalized. Bob received his Bachelor of Arts in Philosophy from Saint Louis University, St. Louis, and his Master of Divinity and Master of Moral Theology degrees from Kenrick-Glennon Seminary, St. Louis. In addition, soon Bob will complete the Master of Arts program in Bioethics and Health Policy at the Neiswanger Institute for Bioethics & Healthcare Leadership at Loyola University, Chicago.
